Care Coordinator

The Fono Trust - the innovator in Pacific health and wellbeing, is introducing a new position at its Manurewa and Henderson practices.

About the Role

The CPCT Care Coordinator is a central member of The Fono’s “Integrated Model of Care” structure which provides a unified wellbeing approach through the services delivered by the units of The Fono. The role is particularly important as it enables coordinated care for those at greatest risk and provides a single plan of care across The Fono services as required, outside agencies. The plan is inclusive of family support and their role in fulfilling it. The CPCT Care Coordinator carries out their duties in the practices and community. The positions based in Manurewa and Henderson are 40 hours per week – Monday to Friday with a competitive salary and professional development options.

About you

The people we are looking for will have:

  • A genuine desire to work with patients, families and colleagues in being able to make a difference.
  • A background in health or social services or suitable experience in an allied field. This could include nursing, social work, family/community services in government, NGO or commercial organisations.
  • A sound understanding and appreciation of Pacific cultures and protocols.
  • Ability develop rapport and communicate effectively with a wide variety of people – colleagues and patients / families.
  • Sound skills in systems, analysis and reporting.
